Local Register - Simplified Template

The Quarantine, Isolation, and Testing Order No.5 (the ‘Order’) has been amended to place additional notification obligations on education facility operators as outlined in the COVID Update # 8 (9 February 2022). 

Schools have been maintaining a local register of all notifications of a confirmed case for staff or students.  You are welcome to continue to use your local register or to use the simplified template shared in the COVID discussion this morning. Note rows 9 and 10 are intended as examples only.  Please provide your school ENumber when sharing your local register with CEB. 

To ensure DOBCEL meets the reporting requirements for Board and Government, all schools in the Diocese of Ballarat are required to email their local register to Catholic Education Ballarat (CEB) via covid@dobcel.catholic.edu.au by 10.00 am each Friday until further notice.  It would be helpful if you can put your school name in the subject line.  

We are happy to receive a copy of your local register (noted above) to avoid duplication of effort. CEB will redact all staff/student names to ensure only de-identified reporting.
